Your next step towards Reinstatement is the completion of an online tutorial. Before you view the tutorial, please keep in mind that:

The full tutorial must be viewed and completed in order to complete the Reinstatement process
The tutorial will take approximately 30 minutes to complete
The tutorial will open in a new browser window. If your browser closes or if the session times out you will be required to restart the tutorial
To enable the tutorial to launch, you’ll need to either turn off your browser’s pop-up blocker or configure your browser to allow pop-ups from PlayOLG.
You will have 30 days from your approval for Reinstatement to complete the tutorial, or your Reinstatement process will be cancelled and you will remain self-excluded
Once the tutorial is complete, you can return to PlayOLG and log into your account
After being reinstated, you will not receive PlayOLG marketing communications unless you update your player profile and opt in to receive marketing

Terms and Conditions of Reinstatement

NOTICE: You have requested and been approved to be reinstated to play on the Ontario Lottery and Gaming Corporation (OLG) Internet Gaming website. Self-Exclusions and Reinstatements apply to all OLG Internet Gaming products (i.e. websites, mobile). You are also required to accept the terms and conditions of reinstatement and complete a mandatory Reinstatement tutorial.

Acknowledgement and Release of Liability

I acknowledge that decisions related to the acceptance or denial of Reinstatement applications are made by employees of OLG in accordance with OLGs policies on reinstatement. I acknowledge that I shall have no claim against OLG or its directors, officers and employees (past and present) arising out of their decision to reinstate me.

I hereby revoke my request to be self-excluded and confirm my request to be reinstated and granted access to OLGs Internet Gaming website and products and services. I acknowledge that I alone have chosen to return to gambling and I was in no way induced to rescind my Self-Exclusion by OLG, by any of OLGs Internet Gaming products and services, by any of OLG's directors, officers or employees, by any of OLG's third-party Internet Gaming service providers, or by any of their directors, officers or employees.

I further acknowledge and agree that OLG, its third-party Internet Gaming service providers and their respective directors, officers or employees (past or present) have taken no other steps to encourage my application for reinstatement, other than to comply with my request. I release and forever discharge OLG, its directors, officers and employees (past and present), its third-party Internet Gaming service providers, any of the third-party Internet Gaming service providers' parent companies, shareholders, subsidiaries or affiliates, or successors, as well as any and all of their directors, officers and employees (past and present), from any and all liability, causes of action, claims and demands whatsoever with respect to my revocation, Reinstatement and re-provision of access to OLG's Internet Gaming products and services.

For greater certainty, and without limiting the generality of the foregoing, I agree that I cannot recover, and I hereby release any claim in respect of any gambling losses that I may incur if I decide to gamble on the Internet Gaming website following my Reinstatement, and lose. I am solely responsible for my gambling.

This Reinstatement Form and Release and the PlayOLG.ca Player Agreement – Terms and Conditions of Use for PlayOLG.ca (the "Player Agreement") constitute my entire agreement with OLG with respect to my request for reinstatement. OLG has not made and I do not rely on any representations to me outside of this form.

Personal Information

Personal information contained on this form is collected and retained pursuant to Ontario Lottery and Gaming Corporation Act, 1999 as well as the The Gaming Control Act. This information will be used to identify you for the purposes of managing the Self-Exclusion program and Internet Gaming operations. This information will be stored in databases by or on behalf of OLG for the purpose of identifying your status as a reinstated person, and will be accessible to those employees of OLG (or its third-party Internet Gaming service providers) responsible for the Self-Exclusion program and Internet Gaming operations. This information will be stored on databases maintained by or on behalf of OLG. You can also contact PlayOLG Player Support at 1-855-978-7529 should you have any questions or complaints about OLG's handling or management of your personal information collected on this form.

I have read this Request to be removed from the Internet Gaming Self-Exclusion List and Release (Reinstatement) and understand all of its terms. I agree voluntarily and with full knowledge of its consequences and significance and hereby request to be removed from the OLG Internet Gaming Self-Exclusion List.

I understand and agree that I am hereby applying to be removed from the Internet Gaming Self-Exclusion list and that I will be permitted to play on the OLG Internet Gaming website, PlayOLG.ca, after completing the following Reinstatement tutorial. If I do not complete this tutorial within 30 days of my approval for reinstatement, my reinstatement process will be cancelled and I will remain self-excluded.
